Wegovy injection guide: sites, steps, and how to use the pen

This is general patient education on Wegovy injection technique — not a substitute for the Instructions for Use in your medication box, and not a replacement for a clinician showing you in person the first time. The steps below explain the how and the why; the leaflet that ships with your pens is the authority on your exact device, and your prescriber or pharmacist can walk you through it. Where to inject Wegovy

Wegovy goes into the subcutaneous fat — the soft layer just beneath the skin, not into muscle. The label approves three sites, and all three work equally well:

Avoid skin that’s bruised, tender, red, hard, scarred, or stretch-marked. Whichever site you pick, the goal is a soft, fatty spot — that’s what “subcutaneous” means, and it’s why lean areas over muscle or bone are off the table.

Injecting into the same square inch week after week can, over time, cause lipohypertrophy — firm lumps of thickened fatty tissue that not only look and feel different but can make absorption less predictable. The fix is simple and it’s the subject of its own section below: rotate.

How to inject Wegovy step by step

The Wegovy pen is a prefilled, single-dose automatic injector — there’s no drawing up from a vial and no separate syringe. The general sequence looks like this, but follow the Instructions for Use in your box for the exact motions for your device:

1. Wash your hands with soap and water.
2. Take the pen out of the fridge and let it come toward room temperature — a cold injection stings more. Don’t warm it with heat; just give it time.
3. Inspect the pen. Check the expiry date and look at the liquid through the window: it should be clear and colorless. Don’t use it if it’s cloudy, discolored, or has particles, or if the pen looks damaged.
4. Choose and clean your site — one of the three above, different from last week’s. Wipe with an alcohol swab and let it dry.
5. Position the pen flat against the skin as the Instructions for Use direct, pinching a fold of skin first if your IFU tells you to for that site.
6. Start the injection per your pen’s mechanism, and hold it in place until the dose finishes — typically until a click and then a few more seconds so the full dose delivers (about 5–10 seconds) (⚠ verify hold time). Lifting early can mean an incomplete dose.
7. Remove the pen straight out, and dispose of the whole thing in a sharps container right away.

Wegovy can be taken with or without food, any time of day — see dosage and timing for the schedule. If you ever feel unsure mid-step, stop and check your leaflet rather than guessing.

Rotating injection sites

Rotation isn’t fussiness — it’s how you protect the tissue that absorbs the medicine. Repeatedly injecting into one spot builds up scar-like fatty lumps (lipohypertrophy) that can blunt and slow absorption, which is the last thing you want from a weekly dose you’re relying on.

A simple system that’s hard to mess up:

• Rotate between the three site areas, and within each area, move to a fresh spot at least an inch from your last injection there.
• Keep a loose pattern. For example, week 1 left abdomen, week 2 right abdomen, week 3 left thigh, week 4 right thigh, then arms — then back around. Any repeatable rotation works; the point is that no single patch of skin takes two doses in a row.

Storing Wegovy

Storage is where a good injection can quietly go wrong, so it’s worth getting right:

• Before first use: keep pens in the refrigerator, roughly 36–46°F (2–8°C) (⚠ verify). Store them in the original carton to protect from light.
• In use / on the go: an unrefrigerated pen can generally sit at room temperature — below about 86°F (30°C) — for up to 28 days (⚠ verify window and temperature), which covers travel and everyday carrying. After that window, discard it even if medicine remains.
• Never freeze Wegovy. If a pen has been frozen, throw it out — don’t inject it (⚠ verify wording).
• Traveling: a small insulated bag or cooler (not directly on ice) keeps pens in range on longer trips; carry them in hand luggage, never checked baggage where the hold can freeze.

Needles & sharps disposal

The Wegovy pen keeps the needle hidden, but it’s still a sharp, and it’s handled the same way:

• One pen, one dose, then done. Each single-dose pen is used once. Never try to reuse a pen or share one — reusing needles dulls them (more pain, more tissue trauma) and sharing risks bloodborne infection.
• Dispose immediately into an FDA-cleared sharps disposal container — a rigid, puncture-resistant, lidded container made for this. If you don’t have one, a heavy-duty household plastic container can be a stopgap, but a proper sharps container is the standard.
• Never put used pens in the regular trash, the recycling, or a flimsy bag.

Common mistakes & when to call your clinician

A short list of the errors that trip people up most:

• Injecting into muscle instead of fat. Pick a soft, fatty site and don’t inject over lean areas — an intramuscular shot stings more and can change absorption.
• Reusing or sharing a pen or needle. Never. One pen, one dose.
• Skipping site rotation, which leads to the lumps that hurt absorption.
• Injecting a cloudy or previously frozen pen, or one past its room-temperature window — inspect first, every time.
• Lifting the pen too early, before the dose finishes delivering.

Injection-site reactions — mild redness, itching, or a small bump that fades within a day or two — are common and usually nothing to worry about. What does warrant a call to your clinician: a reaction that spreads, worsens, or doesn’t settle; signs of infection like warmth, pus, or fever; or any symptom that isn’t about the skin at all — severe or persistent abdominal pain, a lump in the neck, trouble swallowing, or signs of a serious allergic reaction.
